Items that must be provided to enroll a student
  • Proof of Residence                                                                                      (examples: water, light, gas, phone bill, OR real estate property receipt)
  • Immunization certificate
  • Birth Certificate
  • Social Security Card
  • Bring official transcript from previous school (must be in envelope sealed by school)
